I have a LT4120MHD DVD that has recently started erasing added accounts and changed passwords. I have restored the new accounts and passwords but they only last for several weeks and changes everything back to defaults. Any suggestions on how to correct this?

Keep in mind once you do this you will need to reopen the ports for remote access.
I have seen this before. It maybe that the board is failing or the operating system is corrupt. You should have us remote in and do a factory reset on the unit and set everything back to see if that fixes it.

Also make sure you don't have telnet ports forwarded in your router, and at least change the local IP - incase someone is jumping on and wiping the accounts. They can do that with telnet, for example - depends on the DVR/NVR though, newer ones are more difficult. Also as mentioned it could be bad hardware causing the OS to reset (wiping the accounts).
