Items in the kit are the 

POE-SPLIT 1x

PT-6 1x 

MIC 1x

I will be using an IPOD-SB4IR28 for this demonstration.This camera comes with one last piece of the puzzle an RCA Male to a 3-ring (stereo) 3.5mm Connector which we will interface to the Microphone. 

Step 1

Connect the power:

Connect the PoE Adapters DC output to the DC Splitter (PT-6)

Connect the DC splitters output to the DC input of the camera

Connect the other DC splitters output to the MIC's DC Input

Step 2

Audio Connections

Connect the RCA's Side of the RCA to 3.5mm cable to the MIC's Output

Connect the 3.5mm connector to the 3.5mm Input of the Camera Labeled "MIC"

Step 3

Network Connections

Remove any dust cap (*not a weather seal connector)

Connect the PoE Adapters output to the cameras Ethernet Input

Here is an overview of all the connections

The last connection is the PoE Adapters ethernet input to a PoE Switch. 
Once you do so you have completed the MIC Kit connections.

Pro tip > User a small tie to ensure all of the connections stay in a small bundle before putting them inside of your junction box, this helps for later troubleshooting or maintenance. You can also use a multi-use zip tie that allows you to disengage its teeth.
